Transition - Help and Advice

A Promise Inclusionfamily liaison support worker (formerly Bracknell & Wokingham Mencap) can assist the carers of young people with a learning disability (or on the Autism Spectrum) throughout the transition process. They work directly with the family, providing home visits, advice and practical support.  

The Transition Information Network (TIN) is an alliance of organisations and individuals who come together with a common aim: to improve the experience of disabled young people’s transition to adulthood. TIN is a source of information and good practice standards for disabled young people, families and professionals, providing information and resources about transition via their website, publications and events.
